 

HIV prophylaxis with zidovudine, lamivudine and nelfinavir poorly tolerated

WESTPORT, CT (Reuters Health) - Most individuals who receive an HIV postexposure prophylaxis regimen consisting of zidovudine, lamivudine, and nelfinavir (zlnPEP) experience adverse effects, according to a report published in the May 15th issue of Clinical Infectious Diseases.
Dr. Christian Rabaud and colleagues from the Centre d'Information et de Soins sur l'Immunodeficience Humaine in Nancy, Lorraine Champagne-Ardenne, France assessed zlnPEP use in a group of 185 individuals who were possibly exposed to HIV, including 136 healthcare workers.
In total, 106 subjects discontinued zlnPEP before day 28 of the study, either because the source individual had tested negative or because the exposure event was reassessed as being associated with low risk for transmission.
Eighty-five percent of subjects who continued zlnPEP for the full 28 days reported adverse effects associated with the regimen. Just over a third of the patients who completed the full 28-day regimen discontinued one or more drugs. Diarrhea and nausea/vomiting were the most common complaints. Sex, age, or the type of exposure event had no bearing on the regimen's tolerability.
The tolerability of zlnPEP was poor, the authors conclude, even though "the rate of treatment discontinuation was slightly lower than that previously reported with use of the zidovudine-lamivudine-indinavir combination." They urge that "other, better tolerated prophylactic antiretroviral combinations must now be evaluated."
